Using Mobile Phones as Patient Terminals for Home Monitoring

eHealth Home Care Applications: Health Data Management (HDM) is an Internet based concept for mobile and home base acquisition and administration of health related data. Utilising contemporary communication and information technology the HDM system allows to provide and to sustain new connections between patients, physicians and health care institutions. The patients and their health related data are in the centre of interest. Data acquisition via mobile phones, representation, and data analyses are optimised for chronic diseases like hypertension, heart failure, diabetes, etc. The aim is to provide a tool to improve and to optimise the therapy of such diseases using the mobile phone as patient terminal of choice.
